Delineates situations when tests are added to the initial order. This includes reflex and additional tests. 2020-06-06 1996-11-11 The introduction of sensitive thyrotropin assays and free thyroid hormone measurements has simplified the interpretation of thyroid function tests. However, important pitfalls and difficult cases still exist.

A two-site immunoradiometric assay for serum thyrotropin (TSH) was modified to improve the analytical sensitivity. The sensitivity achieved (detection limit, approximately 0.1 microU/ml; lower limit of quantitative measurement, approximately 0.4 microU/ml) was comparable to that of the best competitive Sensitive thyrotropin and thyrotropin-receptor antibody determinations one month after discontinuation of antithyroid drug treatment as predictors of relapse in Graves' disease Thyroid . 2005 Sep;15(9):1047-54. doi: 10.1089/thy.2005.15.1047.
It's important to understand the meaning and possible causes of both high thyroid stimulating hormone (TSH) and low TSH, whether you have been living with thyroid disease for a long time or are only having the test to screen for a thyroid disorder. A high TSH level can mean a new diagnosis of hypothyroidism or inadequate thyroid replacement. A low TSH might mean hyperthyroidism or
American Thyroid Association guidelines for use of laboratory tests in thyroid disorders. We preferred this approach, given that thyrotropin is the most sensitive test to assess response to therapy. 12 We could not reliably ascertain levothyroxine dose adjustments from the OptumLabs Data Warehouse because physician-directed medication adjustments may involve pill cutting and alternate-day dosing. Assessment of sensitive thyrotropin assays for an expanded role in thyroid function testing : Proposed criteria for analytic performance and clinical utility.
More than 40 years ago, thyrotropin (thyroid stimulating hormone, or TSH) was smallest amount of TSH detected (sensitivity) using these assays was 5 to 10
TSH is increased in hypothyroidism and decreased in hyperthyroidism. The new "sensitive" TSH test will show very low levels of TSH when the thyroid is overactive (as a normal response of the pituitary to try to decrease thyroid stimulation). Interpretations of the TSH level depends upon the level of thyroid hormone ; therefore, the TSH is usually used in combination with other thyroid tests such as the T4 RIA and T3 RIA. 1997-03-18 · ELSEVIER Clinica Chimica Acta 259 (1997) 117 128 Discrepancies between thyrotropin (TSH) measurement by four sensitive immunometric assays Ase Krogh Rasmussen~'*, Linda Hilstedb, Hans Perrildc, Erik Christiansend, Kaj Siersb~ek-Nielsene, Ulla Feldt-Rasmussen~ ~ Medical Department P2131, Endocrine Division, Rigshospitalet, D K- Blegdamswj 9, 2100 Copenhagen, Denmark bDepartment of Clinical The sensitive assays for serum thyrotropin 21 now available make it possible to distinguish normal from subnormal values and to identify degrees of suppression of thyrotropin secretion. Thyrotropin-releasing hormone (in its pharmaceutical formulation of 'protirelin') was widely used as a drug to test whether someone had thyroid overactivity. However, there are now more sensitive measurements that can detect very low levels of thyroid stimulating hormone in the blood.

We recommend measurement of the serum TSH thyroid concentration, using a sensitive assay, as the initial step in testing any older person for possible hyperthyroidism. Measurement of the serum T4 concentration or the free T4 index on the same sample would be needed only in the approximately 2% with a serum TSH level of less than 0.1 mU/L; alternatively, the TSH assay in these could be repeated at a later time.

TSH, along with its regulatory hormone thyrotropin releasing hormone (TRH), which comes from the hypothalamus, is part of the feedback system that the body uses to maintain stable amounts of thyroid hormones in the blood. When thyroid hormone levels decrease in the blood, the pituitary gland produces more TSH in response to TRH stimulation.

Some people are exquisitely sensitive to thyroid hormone (including even T4 thyroid doses) which can create strange symptoms that may include both a mix of hyperthyroidism and hypothyroidism. In these cases, it's best to determine if your symptoms coincide with changing your medication and if other factors (such as body temp, heart rate, etc.) imply your thyroid hormone is too high/low.
